Hatte die gleiche Prüfung wie Beitrag #6 von RK. Zusätzliche Fragen:
- Gibt es eine Kinderdosierung für Fluimucil? Welche galenischen Formen (hier wollten sie vor allem Sirup hören)
- Wie verändert sich die Körpertemperatur bei einer Schwangerschaft? (bleibt erhöht) Warum? (optimale Voraussetzung für Entwicklung des Embryos)
- Inhaltsstoffe von Itinerol B6 (Meclozin-HCl und Pyridoxin-HCl). Eigentlich erst zugelassen ab 6 J, was sie hören wollten wurde oben schon beschrieben (also abgeben, solange gut dokumentiert).
- Feniallerg: Welche galenische Form? (Tropfen)
- bei Halsschmerzen: sagt unbedingt, dass ihr Präparate ohne Lokalantibiotika nehmt

Beim Pädiatrie-Teil lohnt es sich wirklich, die markierten Spezialitäten auf den Folien sehr gut zu kennen. Zu jeder Indikation 1-2 Phytotherapeutika ist auch noch gut, die ihr könnt ihr nämlich meistens aufzählen, ohne dass noch weitere Details dazu nachgefragt werden. Es wurden auch einige Fragen gestellt, deren Antworten nicht direkt in den Slides zu finden sind, die aber in der VL erwähnt wurden, hier lohnt es sich also, so gut wie alles mitzuschreiben, was Erni thematisiert.. Grobe Alterslimite zu kennen, ist hier auch hilfreich.

Kleiner Tipp noch: Pädiatrie war auch nicht mein Lieblingsthema und die offene Fragestellung hat mich am Anfnag etwas überfordert :D aber fangt einfach mit einer Indikation an und erzählt dazu, was ihr wisst, ich habe meist auch nicht direkt Präparate genannt, sondern zuerst etwas über die allgemeinen Wirkstoffgruppen erzählt und deren Problematiken bzgl. Kleinkindern hervorgehoben und dann hinterher Präparate zu denjenigen genannt, nach denen er gefragt hat.

Halin: Autoimmunity
Define autoimmunity? (Immune response against self antigens, represents breakdown in self-tolerance mechanisms)
What self-tolerance mechanisms are in place? (wanted to know more about peripheral anergy and how that works, so I talked about co-stimulatory molecules CD80/86, how they are low and not upregulated when we are presenting self antigens, how CD80 and CD86 can bind to both stimulatory receptors [CD28] or inhibitory ones [CTLA-4]; induction of Tregs and activation of T cells; upon CD28 binding: IL-2 as the survival signal)
In the context of IL-2: Is there a drug that one can use in the context of transplant rejection that directly or indirectly targets this? (Cyclosporin and explained the mechanism of action; she then also mentioned Basiliximab and I said that blocks binding of IL-2 to the IL-2R)
Do you know of any genetic mutations that can cause autoimmunity? (Many, I just went for my favourite - AIRE => APECED, explained negative selection in thymus, what AIRE does and what happens if it doesn't function properly).

Neri: Viral quality control
What do we do with the cell culture supernatant? (TEM: talked about how that is the only method where we don't rely on any hypotheses/assumptions as to which viruses might already be present; also mentioned endogenous retroviruses)
MAP, HAP, RAP and inapparent virus assay (just explained everything from the slides and mentioned again the fact that for the ELISA in MAP/HAP/RAP, one needs to already suspect certain viruses and use those antigens to coat the wells)
Downstream processing: We miniaturize our system and perform certain tests with it, can you tell me which? (Viral reduction factor; explained the process, also mentioned that one can simply add up the log-numbers for the 3 orthogonal methods which he liked very much :))
Last question: GMP facilities. What role do they play for the control of viral contamination? (This took me a bit by surprise, but I just said that it is not just about the production process itself, the facility also plays a role, mentioned that for some parenteral drugs we do sterilization or produce them under aseptic conditions, that the generation of MCB has to take place in a GMP facility; he added that there are these classes (Reinraumklassen A-D, flashback to Qualitätsmanagement from 6. semester BSc ^^) but he already seemed content with the answer so he probably didn't expect you to elaborate that much on this.

I think my exam ended after about 10 mins. They were really nice, encouraging, nodding and smiling all the time. Some people have mentioned that they prefer short answers, but I found that they never interrupted me when I elaborated on a topic as long as that was still relevant to the questions they asked.

Fach: Radiopharmazeutische Chemie

Schibli hat angefangen mit Screensharing und ein Bild der Karlsruhe Nuklidkarte gezeigt und gefragt, was das ist. Dann sollte ich ein bisschen etwas erklären zu den verschiedenen Zerfallsarten, anhand der Karte bestimmen, zu was zB 14N zerfällt. Welche man für Therapie und Diagnose einsetzt und warum (habe hier auch LET erwähnt). Dann hat er gefragt, ob man auch b+ für Therapie einsetzen könnte (b+ und b- haben dieselbe Masse UND dasselbe LET - deshalb theoretisch ja!) und dann haben wir noch ein bisschen über FDG gesprochen, warum man es nicht als Therapie einsetzen kann (Neurotoxizität, da GLUT1 im Hirn), wieso es in Tumorzellen aufgenommen wird (Warburg Effekt). Dann kam Fr. Mu's Teil: allgemein Herstellung von 11C und 18F erklären, dann hat sie Screen geshared und ein Molekül gezeigt und ich sollte sagen, welche Stellen ich wie labeln würde (denkt dabei nicht nur an Methyliodid sondern auch an Grignard ;)) und das war's auch schon. Danke an alle, die bisher ihre Erfahrung geteilt haben, und viel Glück allen, die es noch vor sich haben :)
